NDC: 54288-800-01 1 vial per package, 2 mL single dose vial.  This is a Compounded Drug. 400 units/2 mL (200 units/mL) For Intramuscular and Subcutaneous Injection Each mL contains: Calcitonin Salmon, USP 100 units, Acetic Acid USP 1.125 mg, Phenol USP 2.5 mg, Sodium Acetate trihydrate USP 1.0 mg, Sodium Chloride USP 3.75 mg, Water for Injection.
