New-Client Consultation

Peace and welcome to all of the new visitors and clients of Naturally Euphoric! You are here because you're not quite sure what it is that you need, but know that you need something, and a consultation can get you started in the right direction! Take a few moments to fill out this questionnaire and then our herbalist will reach out to you within 2 business days.  Please be sure to answer all questions honestly to the best of your ability, as it will ensure the best and safest help possible. All of your health information is protected by the HIPPA laws.  Consultations generally take between 20-45 minutes, depending on the person. Consultation fee will go towards any products purchased based off of your consultation recommendations.  Here at Naturally Euphoric, we are NOT doctors and we cannot make diagnosis.
